Certificate of Use

A Certificate of Use (CU) is required by Section 3-17 of the Town of Cutler Bay Code of Ordinances. These CUs are required to confirm that a proposed use conforms with the standards of the Town's Land Development Regulations. The application form for a CU can be found here. Below is a checklist of documents that will be required for the Department to process your CU application:

Certificate of Use Application Checklist

  • Cutler Bay Use Request Form and Miami-Dade County Municipal Application for Certificate of Use/Occupational License (Approved by the Planning and Zoning Division prior to application).

  • Copy of Miami-Dade County Approval of Municipal Application for Certificate of Use or Business License placard. (Contact DERM – 786-315-2800.)

  • Completed Cutler Bay Certificate of Use Application form and payment of $35.00 fee. (May be submitted by check or money order made payable to "Town of Cutler Bay").

  • Federal Tax ID Number or EIN (this is obtained from the Internal Revenue Service or 800-829-4933).

  • State of Florida Business Registration Documents this can be filed Articles of Incorporation, LLC registration, or Fictitious Name. Available through www.sunbiz.org.

  • State/Trade License (if applicable based on the type of business being registered.)

  • Fire Permit Certificate/Fire Inspection Report available through the Miami-Dade Fire Department (www.miamidade.gov/fire), 786-331-4800.

  • Lease Agreement including signatures of the landlord and tenant (Home Based Businesses must have property owner approval if the applicant is a tenant.)

  • Parking Plan/Site Plan indicating the location and number of parking spaces provided.

  • Home Based Businesses

    • Floor Plan of Residence indicating the location and dimensions of the home office.

    • Home Occupation Affidavit (available on the Town’s website).

  • Cutler Bay Local Business Tax Receipt Application, found on the Town’s website (fee depends on business type, please contact the Town at 305-234-4262 and dial 7 for more information).

  • Miami-Dade County Local Business Tax Receipt, contact by phone at 305-270-4949 or www.miamidade.countytaxes.com/btexpress.

  • AHCA Licensing Certificate for Assisted Living Facilities (Contact: 850-412-4304).

  • Certificate of License from Agency for Persons with Disabilities for Group Homes (Contact: 850-488-4257).
